Everyware Health Care: Real Life Examples

I'm devoting some of the posts I write for this blog on the topic of "Everyware Health Care", an over-arching concept that emphasizes how existing and emerging technologies can help to improve delivery of health care and also enhance health-care user experience.

About 182,000 patients at the Cleveland Clinic use MyChart, an in-house electronic medical record. More recently, the hospital is testing out Google Health Records with 1,500 patents and Microsoft's Health Vault with 500 patients. According to an article in the Plain Dealer, the hospital supports the use of electronic medical records because it can reduce costly mistakes and redundancies.

Checking blood pressure






The video above shows how blood pressure can be charted and tracked daily, providing the physician with information that can better monitor and adjust medications. With Microsoft's Health Vault system, patients can upload medical information via a glucometer, pedometer, weight scale, or blood pressure monitor, depending on the nature of their health concerns. The system provides a means for uploading information to the system wirelessly, which is important for people who travel a lot for their jobs.

My Family Health Portrait is an on-line tool provided by the U.S. Department of Health and Human Services that allows people to enter their family health history, create drawings of family health history to share with health care providers or other family members, and to integrate the health history of other family members into your own. The source code for developers is available for free.

Some of the basic work related to integrated electronic medical information systems is hospital-focused Microsoft's Amalga software, which has been rolled out in several hospitals around the world. The following video shows how the system works at Bumrungrad International Hospital, and how it improves the hospital user experience, not only for the patient, but for the medical staff as well:

"Everyware" Health Care: A Personal Focus



For the next few weeks, I'll be focusing some on my experiences with technology and health care.

This time, my posts will be on a more personal level. As I write this post, I am sitting in the family waiting area of the cardiac ICU at the Cleveland Clinic. My dad's surgery was Tuesday, and it is now Sunday, so I have had plenty of time on my hands to observe how technology is used to deliver his care, monitor his progress, and support his recovery.


Since I view things from a broader human-computer interaction perspective, I've decided to share some of my thoughts the"Hospital UX" picture, putting myself in the shoes of patients, family members, and medical professionals.


A Clean and Sterile Wait

This is the new lounge where families wait while their loved ones undergo cardiac surgery. The old family waiting area at the Cleveland Clinic had a cozy, home-like feel, with comfortable chairs and cheery art. The new waiting area has a very sterile, minimalist feel. The chairs are NOT comfortable, especially if you are middle-age or older and have the usual aches and pains!

Internet of People, Things, Surfaces, and Interactions: Insights from UX 2008 & Adaptive Path

"Create the world, the interface will follow" - Link to article by Adaptive Path's Paula Wellings

Paula Welling's article, "Create the world, the interface will follow", draws from her observations as an experience designer at Adaptive path and her previous work as a creator of imaginary game worlds. She points out that it is possible for user-centered designers to focus so much on people, via persona development and contextual inquiring, that the role of the real-world environment is not fully explored.

Wellings asks experience designers to look at the bigger picture.

"How can we move from a purely descriptive representation of the people themselves to an approach that explicitly recognizes design as facilitating participation in particular worlds?"

Here is a quote from the article:

"Like the worlds of video games, our real world is a possibility space* that gives depth and context to our interactions. For the most part, people live their lives in environments that provide structure to activities, relationships and opportunities. Changes of environment reveal the power of the world to enable and diminish our possibilities. Both dramatic environment changes such as living in a foreign country, going to jail, and surviving a natural disaster as well as small changes such as moving from a sunny climate to a rainy one can affect people in powerful ways."

As an observer of human-world interaction, I agree with Wellings' point of view.

Why not adopt the Google User Experience Team's design principles for off-the-desktop digital interactions?

If you haven't noticed yet, interactive urban (and suburban) screens of all shapes and sizes are found outside the home and workplace, in places we've come to expect, and in unexpected places. Our cell phones have morphed into web browsers, with larger, higher resolution screens.

The web and the real world have become intertwined.

But often, it is not pretty.*


It is as if all of the blocked pop-ups, banner ads, and fast-forwarded commercials escaped, and found new homes on the screens we find staring at us in airports, malls, arenas, and big-box store aisles!

Is it possible for user experience principles to enhance our digital lives beyond our laptops and desktops?

A recent post on the Putting People First blog outlines some great advice from the Google User Experience Team that holds true for off-the-desktop applications, including multi-touch displays, digital signage, kiosks, mobile devices, and anything under the umbrella of DOO- digital out-of-home:

"The Google User Experience team aims to create designs that are useful, fast, simple, engaging, innovative, universal, profitable, beautiful, trustworthy, and personable. Achieving a harmonious balance of these ten principles is a constant challenge. A product that gets the balance right is “Googley” – and will satisfy and delight people all over the world."

The ten principles that contribute to a “Google User Experience” are:

1. Focus on people – their lives, their work, their dreams
2. Every millisecond counts
3. Simplicity is powerful
4. Engage beginners and attract experts
5. Dare to innovate
6. Design for the world
7. Plan for today’s and tomorrow’s business
8. Delight the eye without distracting the mind
9. Be worthy of people’s trust
10. Add a human touch

Off-the-desktop musings about future interactions: User experience, user-driven design, Universal Usability, Airports, and the "Internet of Things"

My vision for technology supported human-world interaction is much different than what I see cropping up in public spaces, despite emerging technological concepts such as the Internet of Things and Locative Media

If you are familiar with my blogs, you know that I maintain an on-going quest for off-the-desktop examples for my "Usability Hall of Fame". While I'm out and about, I often meet with disappointment, especially when I come across large displays or interactive touch screens.

The video clip below, taken on June 26th at the Detroit Metro Airport, is an example of what I mean. I was excited to encounter a touch screen information kiosk. I was not impressed with the clunky look and feel of the application. It was more like a point and click web-page than an innovative interactive information portal.



If you look closely, you'll see that the weather posted on the screen did not accurately represent the near 90-degree heat that I encountered that day. The weather information must not have been updated!

This kiosk was located in a new terminal. Although there were many information displays dotted around, during the two hours that I was at the airport, I never once saw anyone use them.


















It is sort of difficult to use a touch screen when there is a garbage can obstructing the interaction.





















It is not possible to interact with an interactive touch screen that is not working.


I did see people take a look at the more traditional, non-interactive displays that were located in the center walkways:



















Last week, when I went to find my baggage at the Detroit Metro Airport, I noticed three kiosks on my way to pick up my luggage. At the time, I didn't know where I needed to go to retrieve my bags, and I wasn't sure where I needed to go to take the shuttle to the rental car lot. I was a bit puzzled to find these kiosks in this spot, since they did not provide the kind of information that most people coming through this section of the airport would need.



































The above information kiosks were situated in a row of three, one representing each of the counties that make up the Detroit metropolitan area. Since the kiosks were located at the foot of the escalators leading to the baggage claim section of the airport, people stopping to interact with the kiosks would most likely cause a minor traffic jam. Although the kiosks were in plain view, it was not difficult to see why they were not used, or even noticed.
  • Most people who are on their way to the baggage claim area are focused on getting their luggage and heading out. They may not be in the mood to learn all that they can about the finer points of Macomb, Wayne, or Oakland counties from talking head infomercials.
  • Although the kiosks provided a reasonable amount of screen "real estate", the design of the applications allowed for no innovative touch-screen interaction. The applications were displayed on touch-screen surfaces, with the upper section dedicated to video and the lower section activated for touch. This design provided only one type of touch interaction, a small area to touch in order to activate the video.
  • The displays with information about baggage carousels were located in a spot out of view from this location.
After I figured out where I should go to find my luggage, I noticed that there were many lighted displays lining the walls behind the baggage carousels. People were gathered around the carousel, eyes searching for bags. No one was looking at the signs.

















Since I'm obsessed with displays, as soon as I found my bag, I walked up closer to see what was so important about this huge sign. Not until I got right up to the surface did I notice that it offered some interactivity:















If you happen to have your cell phone handy, you can get free sync ringtones from the display area while you wait for your bags, if you happened to notice this at first glance. Since I was carrying my purse and pulling a suitcase and smaller carry-on bag, I was in no position to fumble around for my phone to get a new ringtone. It wasn't a convenient location. Where could I set down my purse? What if I had kids in tow?


Obviously, the designers of this set-up thought that it would be a good idea to offer people the chance to download ringtones as they waited for their luggage. What did the designers forget? Perhaps they did not look at the bigger picture long enough to think through a workable, useful user experience.

It would make better sense to provide ringtones at a more convenient location in the airport, such as the waiting areas or around the food courts. A thoughtful re-design of this display would encourage this interaction in a way to let people know intuitively that a ringtone service was available.

After I left the baggage area, I headed to an information display that consisted of a small touch screen, an array of small lighted displays, and phones to find out how to get to the shuttle to the rental car lot. Again, I was not impressed. The applications were a bit more visually appealing than the displays in the terminal, but the touch screen interaction was clunky.

World Usability Day's Healthcare Focus: Thoughts about health care user-experience design...

The theme of the 2007 World Usability Day is healthcare. This event aims to create awareness about the need for user-centered design within the health care system. Here is some information posted on UPA's World Usability Community blog:

"..."The importance of user-centered design in healthcare is truly about life and death” noted Elizabeth Rosenzweig, Founder and Director of World Usability Day. “Whether it’s new medical devices or technologies; drug research, approval or delivery; patient forms or medical record sharing; emergency disaster planning or increasing the functionality of hospitals and everyday healthcare delivery, everyone is effected in some way by the intersection of usability and healthcare. There are many commonalities, yet each region of the world faces its own set of unique challenges. We believe that focusing World Usability Day 2007 on healthcare will create a stronger awareness of these issues and lead to initiatives that have long term impact on the quality of everyone’s life".

Usability and interaction with the health care system has become a personal interest of mine, as I've spent the better part of the past month assisting a close relative negotiate three hospital stays, a couple of surgeries, numerous medical tests, and visits to various medical professionals. This has been an eye-opening experience, as anyone who has followed a similar path would agree.

I propose that efforts to improve usability should consider using some of the strategies employed by user-experience designers, taking into consideration that the definition of "user" includes the patient/consumer, the patient's family and social support network, health care professionals, hospital and medical staff, health educators, researchers, and so forth.

When I have the chance, I'll post some of my observations and thoughts related to this topic.
